Effective 01 January 2013, the Maryland Order for Life Sustaining Treatment (MOLST) form replaces completely the now former EMS/DNR form. Maryland MOLST is a portable and permanent (until you change your instructions) medical order covering options for CPR (cardiopulmonary resuscitation) and other life-sustaining treatments. The medical orders are based on your wishes for medical treatments. Please note the MOLST is an Order from your doctor. It is extremely important that you or your health care agent, make sure that your doctor completes the MOLST to reflect your wishes, not those of your doctor nor of any other person.
